Dentists that participate in a dental network agree to accept negotiated fees as payment in full for covered services. Canopy members have access to the DentaNet network in Alabama and the DenteMax network in all other states. Outside Alabama, if your provider accepts DenteMax, they accept Canopy Insurance.
You will generally save money if you go to an in-network dentist.

- For Alabama residents: DentaNet includes a wide range of dentists who are contracted to provide dental services at a negotiated rate within Alabama.
- For members outside of Alabama: DenteMax is a national dental network that offers access to a large selection of dental care providers across the United States.

To view your specific benefits, you can access the Canopy Insurance Member Portal, which provides detailed information about your coverage.
Online Access:
- Register on the portal to create an account: If you haven't created an account yet, you can easily set one up by clicking on "Register" to Register as a Member/Individual. You will need your Contract Number, which is located on your Canopy ID Card.
- Log in: if you already have an account, log in to the Canopy Insurance portal to see a summary of your benefits. This includes your covered services, copays, deductibles, and out-of-pocket maximums.

- Tell your provider that you have VisionPlus powered by VSP™ Vision Care.
- An ID Card is not necessary, but you can access your ID Card on vsp.com. Without an ID Card, the provider will need the patient's name, date of birth, and the last 4 digits of the Primary Insured's Social Security number.
Yes. If you go to an out-of-network provider, you may still be eligible for benefits. Call 800-877-7195 to see what those benefits may be.
If you go out-of-network, you can submit a claim for reimbursement within your account on vsp.com or call VSP™ at 800-877-7195 to request a claim/reimbursement form.

Accident & Critical Illness
